WHAT ARE SPECIALIST PROCEDURES?

Advanced treatments for more complex conditions

Some conditions require a more targeted or technically demanding approach than a standard joint or tendon injection.

Our specialist procedures use ultrasound in real time to identify the exact structure being treated, plan the approach and guide treatment accurately.

These procedures are offered for selected patients following clinical review. Suitability depends on your diagnosis, previous treatment, imaging and individual circumstances.

OUR SPECIALIST PROCEDURES

  • SHOULDER

    Frozen shoulder, calcific tendinopathy and selected tendon procedures

    Hydrodilatation

    For selected cases of frozen shoulder / adhesive capsulitis. Fluid is introduced into the shoulder joint under ultrasound guidance to distend the tightened joint capsule and help improve pain and movement.

    Barbotage

    For calcific tendinopathy of the shoulder. Ultrasound-guided needling and lavage is used to target calcium deposits within the affected tendon.

    Tendon fenestration / dry needling

    For selected persistent shoulder tendon problems where appropriate. The tendon is targeted under ultrasound guidance to stimulate a healing response.

    Other complex shoulder procedures

    Additional image-guided shoulder treatments may be available following clinical review.

  • HAND & WRIST

    Trigger finger, ganglion, carpal tunnel and small-joint procedures

    Trigger finger procedure

    For trigger finger / stenosing tenosynovitis. Ultrasound-guided treatment targets the affected tendon sheath and the area of restriction.

    Ganglion aspiration

    For selected symptomatic ganglion cysts. The ganglion is identified using ultrasound and drained where clinically appropriate.

    Nerve hydrodissection

    For selected nerve entrapment conditions, including some carpal tunnel presentations. Fluid is introduced around the affected nerve under ultrasound guidance to separate it from surrounding tissues.

    Small joint injections

    For selected painful or inflamed joints of the hand or wrist. Ultrasound guidance can be helpful when treating smaller or technically difficult targets.

    Selected tendon sheath procedures

    For selected tendon sheath conditions where specialist ultrasound-guided treatment is appropriate.

  • HIP & GROIN

    Trigger finger, ganglion, carpal tunnel and small-joint procedures

    Deep hip joint injection

    For selected hip joint pain and inflammatory or degenerative conditions. A deep ultrasound-guided injection into the hip joint using real-time imaging for accurate needle placement.

    Iliopsoas bursa / tendon injection

    For selected anterior hip or groin pain associated with the iliopsoas tendon or bursa. Ultrasound allows precise treatment of these deeper structures.

    Proximal hamstring procedures

    For selected proximal hamstring tendinopathy. Ultrasound helps accurately target the deep hamstring origin.

    Other deep hip / groin injections

    Selected other technically demanding image-guided procedures may be offered following review.

  • KNEE

    Patellar tendon procedures, high-volume injection and Baker's cyst aspiration

    Patellar tendinopathy procedures

    For selected persistent patellar tendon problems. Specialist ultrasound-guided tendon procedures may include fenestration / dry needling or other targeted treatment depending on the diagnosis.

    High-volume injection

    For selected patellar tendon conditions where clinically appropriate.

    Baker's cyst aspiration

    For selected symptomatic Baker's cysts. Ultrasound is used to identify the cyst and guide aspiration where clinically appropriate.

    Other complex knee procedures

    Additional specialist ultrasound-guided knee treatments may be available following review.

  • ANKLE & FOOT

    Achilles, subtalar, plantar fascia, Morton's neuroma and complex foot procedures

    Achilles tendinopathy procedures

    For selected persistent Achilles tendon problems. Specialist ultrasound-guided tendon treatment may include

    fenestration / dry needling or other targeted procedures.

    High-volume Achilles injection

    For selected Achilles tendon conditions where clinically appropriate.

    Subtalar / complex ankle injections

    For selected ankle, hindfoot and subtalar joint conditions. Real-time ultrasound helps access smaller or deeper joints

    accurately.

    Plantar fascia procedures

    For selected persistent plantar fascia pain. Ultrasound allows the plantar fascia and surrounding tissues to be

    accurately identified and targeted.

    Morton's neuroma injection

    For selected symptomatic Morton's neuroma. Ultrasound guidance allows accurate treatment around the affected interdigital nerve.

    Selected small joint injections

    For selected painful or inflamed joints of the foot where image guidance is required.

  • THORACIC OUTLET & OTHER SPECIALIST PROCEDURES

    Scalene muscle procedures and selected complex image-guided treatments

    Scalene muscle block

    For selected patients being assessed or treated for neurogenic thoracic outlet syndrome. A targeted ultrasound-guided injection is placed into the scalene muscle as part of specialist assessment or treatment.

    Targeted diagnostic injections

    For selected situations where there is uncertainty about which structure is causing pain. A precisely placed local anaesthetic injection can help determine whether a suspected structure is contributing to symptoms.

    Other complex image-guided procedures

    Additional specialist procedures may be available following review of your diagnosis, clinical information and imaging.

  • No. You do not need to choose the procedure yourself. Our clinical team will review your symptoms, diagnosis and imaging and help determine whether a specialist procedure may be appropriate.

  • Yes. Ultrasound is used in real time to identify the target, plan the approach and guide the procedure.

  • Some joints, tendons and other structures are deeper, smaller or technically more difficult to access. These procedures require specialist image-guided expertise and precise ultrasound guidance.

  • No. Suitability depends on your diagnosis, imaging, previous treatments and individual clinical circumstances. Sometimes another treatment, further imaging or specialist opinion is more appropriate.

  • This depends on the procedure, available imaging, clinical review and appointment planning. Some patients require further assessment or imaging before treatment can be arranged.

  • Aftercare varies depending on the treatment. We will explain any activity restrictions, rehabilitation, follow-up or further treatment you may need.
